- Collation of Data for Analysis
- The Charting of Causal Factors
DAY 3 (morning)
- Developing and Implementing Corrective Actions
- Economic Considerations
DAY 3 (afternoon)
- Example Case Studies (short), not connected with the delegate's place(s) of work
DAY 4
- Case Studies related to the delegate's place(s) of work
DAY 5
- Case Studies related to the delegate's place(s) of work
- Review of what has been learnt and Action Plans to implement this on returning to work

Course Content
Aim:
1) To enable delegates to investigate, categorise and ultimately eliminate the root causes of incidents. 2) To enable delegates to generate specific recommendations for preventing incident recurrences.
Pre-Requisites:
Prior to the course start date, delegates will be required to provide non-confidential data from their place of work in order for the Course Tutor to design relevant case studies to be used as part of the Training Course.
Duration:
Five days.
Optimum Number:
Ideally, a minimum of eight delegates (2 teams) and a maximum of sixteen delegates (4 teams).
Training Aids:
The use of a “Root Cause Map”.
